Ender 3 V3 SE layer shift after about 40 minutes

The shift is always on X and the printer sounds normal before it happens. Belt feels snug and the pulley grub screws appear tight. Could acceleration be enough to cause a single big shift, or should I be looking for cable snagging first?

A cable snag can absolutely create one dramatic shift. With the printer off, move the toolhead through the full X/Y range and watch the loom. If that is clear, temporarily reducing acceleration is a useful A/B test.

Also put a paint mark across the motor pulley and shaft. A grub screw can feel tight but still let the pulley slip intermittently if it is not seated on the flat.
